About the unit The Corewell Health Center for Diabetes & Endocrinology provides a wide range of specialized services for patients in West Michigan and beyond. We emphasize education, treatment, and prevention of complications for disorders of the glands, hormones and metabolism. Easy, coordinated access is provided to specialties and services, including heart care, foot and ankle medicine, neurology, eye care, weight management and pain management. We work with patients to explain the importance of exercise, nutrition and blood sugar control for regulating diabetes and improving quality of life. We test the blood glucose levels of everyone admitted to our hospitals. This tells us if patients have diabetes mellitus or if they’re at increased risk for developing it (pre-diabetes). Well-controlled blood glucose levels have been shown to lead to fewer complications, improved healing and shorter hospital stays.About Outpatient Clinics Corewell Health operates over 300 outpatient clinics across Michigan, bringing a multitude of care options to the communities that we serve. There are over 40 different types of primary care and specialty practices, which allows those who work in these clinics the opportunity to learn and explore various interests or to specialize in one area. Scope of work Serves as a population health subject matter expert in the practice. Plans and prioritizes care for individuals and population of patients, focusing on strategies that will promote optimal health within populations and directs patients to the right care team member at the right time. Demonstrates expertise, current knowledge in nursing care and management of a sub population of patients of varying complexity and seeks to improve patient, family and health systems / community outcomes through the application of educational concepts / skills and preventive care in a managed care environment.How we will care for you, while you care for our patients

Qualifications
Bachelor's Degree or equivalent nursing or a healthcare related field
3 years of relevant experience clinical nursing and or case management/managed care or related field required
LIC-Registered Nurse (RN) - State of Michigan Upon Hire required
CRT-Basic Life Support (BLS) - AHA American Heart Association or American Red Cross Upon Hire required
