Responsibilities Forest View Hospital, located just seven miles southeast of downtown Grand Rapids, Michigan, is a private 108 bed psychiatric facility that serves children, adolescents and adults. We are licensed by the State of Michigan, fully accredited by The Joint Commission and we bring more than 45 years of experience to the evaluation, diagnosis and treatment of a wide range of behavioral health problems. As a part of our continued success and growth, we are seeking qualified applicants for a Discharge/Care Coordinator position. Visit us online at: https://forestviewhospital.com/ The Discharge/Care Coordinator provides discharge planning support to the psychiatric case-management department, as well as helping with additional social services duties. Key Job Duties and Responsibilities:
Initiates discharge planning and arranges aftercare appointments/referrals.
Completes all needed discharge paperwork with patients and supports at time of discharge.
Completes psychosocial assessments on new patients within the required timelines.
Engages in collateral contacts with community organizations, support persons, and other providers and appropriately documents the contacts.
Attends appropriate team meetings as needed.
Other duties as assigned.

Qualifications
Bachelor’s degree from an accredited college or university in Social Work preferred; Current social work students will be considered.
One-two years of pervious behavioral health experience preferred.
Ability to multi-task and manage multiple priorities.
Ability to remain calm and professional under pressure.
