About the Role Like their hospital counterparts, the ACRN will be responsible for starting the patient’s care which includes performing a comprehensive nursing assessment, medication reconciliation, safety assessments, and implementing standardized protocols. The difference is the ACRN partners with a dedicated hospitalist team, a Recovery Care Coordinator, as well as community paramedic partners and suppliers to provide comprehensive, timely, hospital level care in the comfort of the patient’s home! After the start of care visit, patients receive ACRN visits at least 2 times per day. During these visits, the ACRN will assess the patient’s condition, administer prescribed medications (typically intravenously), obtain lab samples, provide therapeutic interventions, and educate the patient and family members on how to manage their illness; all with a goal of restoring the health and stability of the patient. During the morning visit, the ACRN will participate in virtual rounds with the hospitalist provider and patient. The Recovery Care Coordinator will work remotely with the ACRN to assure needed supplies are at the patient’s house, arrange transportation and follow-up appointments, and follow-up with the provider as labs, tests, and imaging results become available. This is a great opportunity for an experienced RN that enjoys connecting one on one with patients while providing highly skilled care! Because our ACRNs provide hospital-level care in the home, 1 year of inpatient or home health care experience is highly desired, and experience providing nursing care in the home, community, or clinic is very beneficial. Current license RN license in State of Michigan and reliable transportation are required. Over IRS Mileage Reimbursement rate. Primary Responsibilities

Perform patient assessment and provide care in the home

Educate patient on their condition, prescribed medications, nursing interventions, and care plan

Participate in the coordination of services with other disciplines and community resources as needed

Identify and act on changes in patient’s condition

Evaluate and revise care plans/assessments based on changes in patients and/or their environments

Document all assessments and interventions

Collect specimens as required (including phlebotomy)

Start and manage peripheral IVs.

Manage midlines and PICCs

Administer medications including IV medications as ordered

Insert foley catheters as ordered

Utilize and instruct patient/family on the use of the telehealth tablet used for biometric data capture, patient symptom reporting, and telehealth video visits

Participate in regular case conferences and in-service or special case orientations as needed and participate in on-call rotation

Other duties as assigned.

Requirements

ASN required, BSN preferred

Currently licensed to practice as a Registered Nurse in the State of Michigan

1 year of inpatient experience or home health care as a registered nurse desired

Additional experience providing RN services in the home, community, or clinic is preferred

RN would be seeing patients within a 20 mile radius of the Detroit Campus

Work Schedule: Flexible Schedules available!

Preferred 10 hour shifts, Noon - 10:30pm.
