The Town of Rochester Council on Aging is seeking a part-time Outreach Coordinator for up to 19 hours per week at $23.76 per hour. JOB DESCRIPTION

TITLE: Outreach Coordinator – Council on Aging

SUMMARY: Reporting to the Council on Aging Director. The primary function of this position is to assist elders and provide support to their families in defining needs, facilitating access to services, and providing information and referrals in a timely manner, abiding by all municipal, state and federal guidelines, regulations and quality standards and ensuring confidential, safe, courteous and professional behavior is observed in all situations.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Reviews email, voice mail and other communications, checks schedule / calendar for meetings and appointments.

Attends required scheduled meetings, training sessions (day and evening) and client appointments as required.

Attends community events to disseminate information regarding elder services and issues.

Performs direct counseling and crisis intervention (medical / social / financial) with clients by phone and in person at the office and provides home visits as needed.

Conducts home visits, assists seniors in defining needs, facilitates access to services, makes appropriate referrals and provides assistance.

Conducts assessment of clients; reviews and determines case management plan; coordinates and implements delivery of services.

Communicates with client families, agencies and other organizations regarding referrals and the need for information and counseling.

Manages and conducts the distribution of resources to clients, as needed (including shopping cards, food distribution, Farmer’s Market coupons and food baskets and other related issues).

Responds to crisis; coordinates with local agencies and safety departments to modify or alleviate crises.

Advocates and intervenes on behalf of clients to help ensure access to needed services, resources or facilities (public or private).

Provides advocacy and works with SHINE in accordance with COA / Director’s policies in legal, social security, food stamps, health, housing, medical and fuel assistance matters as required.

Seeks new clients, solicits and accepts referrals from private individuals, social service agencies, religious and community organizations and the police or other relevant entities.

Compiles and maintains all necessary program reports and records; submits monthly statistical report to the Council on Aging Director.

Prepares and submits, as applicable, grant-related forms and related reports. Provides feedback to and collaborates, as applicable, with the Council on Aging Director regarding the needs of clients.

Demonstrates high regard for patron privacy and the policies and procedures of the COA, maintains strict confidentiality of client information, to be shared only with the Director and appropriate authorities, as determined by or in collaboration with the COA Director.

Complies with mandated reporter guidelines and procedures and reports information to the appropriate Protective Services agency.

Assists in the development of programs that will benefit the elder population and the community.

Performs other duties as assigned by the Executive Director of the Council on Aging.

MINIMUM QUALIFICATIONS

Education and Experience: Minimum of an Associates / Bachelor’s degree in human / social service work or related college coursework with MSW preferred. Must have (2-3) years of outreach / geriatric / human services experience in related field or equivalent combination of education, experience and training which provides the required knowledge, skills and ability to successfully perform the duties of the job. Must have the ability to effectively work with and understand the needs, issues and concerns of the client base of the COA (elders/ disabled and the physically-mentally challenged of all ages and social-economic standing) in a sensitive, definitive, respectful, compassionate and professional manner. Must have a valid Massachusetts driver’s license and full time access to a vehicle with appropriate insurance coverage. Must have or complete CPR and First Aid training and ensure certification is up to date for the duration of employment. Must have good computer skills, fluent in Microsoft Office software, internet applications or like operating systems. Must demonstrate high regard for patron privacy and the policies and procedures of the Council on Aging. Must be able to work collaboratively with COA Director and applicable agencies.

Must be able to perform all job functions safely.

Must be able to work the scheduled / assigned times for the position.

Must submit to mandatory drug testing.

Must submit to a background / CORI check and not possess significant criminal record, as determined by the appropriate authorities.

Must have the ability to respond to inquiries/complaints with tact and discretion, work calmly / effectively.

Must demonstrate good judgment and have the ability to work independently and maintain client confidentiality.

PHYSICAL DEMANDS:

While performing the duties of this job, employee may be required to hand lift up to 50 lbs occasionally, hand carry up to 50 lbs occasionally. Employee must be able to stand, sit, or walk for extended periods of time.

WORK ENVIRONMENT:

This job is performed in a generally clean and healthy indoor office environment.
