When you join the growing BILH team, you're not just taking a job, you’re making a difference in people’s lives.Job Type:RegularScheduled Hours:32Work Shift:Day (United States of America)32hpw; Day Shift 8am-4:30pm; Rotating weekends and holidaysJob Description:Duties/ResponsibilitiesProvide clinical intervention, including psychosocial, risk and level of care assessments, for patients identified as meeting high risk criteria by Care Team members. Complete the Case Management Initial Assessments for patients assigned to Social Work upon admission. Provide consultation to members of the Care Team on bio-psychosocial, environmental and cultural factors. Provide brief therapeutic intervention with patient and families around the issues of diagnosis, adjustment, grief or loss. Utilize appropriate case management interventions to address current and/or future barriers to care and well-being; facilitate referral to support services to promote well-being and manage length of stay. Upon patient admission, initiate the process of developing a discharge plan with patient, family and medical interdisciplinary team which will result in a safe disposition for the patient. Facilitate family and treatment team meetings early in the admission, as clinically indicated. Participate in supervision and team meetings. Adhere to National Association of Social Work ethics and scope of service standards for Social Work Case Management practice.It is understood that this is a summary of key job functions and does not include every detail of the job that may reasonably be required.Experience/Knowledge/Skills Required

Four years of Social Work experience within healthcare setting, psychiatric and substance use.

Knowledge of community resources

Effective verbal and written communication skills

Person-centered engagement skills

Ability to build relationships with patients, family members, community collaborators and colleagues

Brief treatment and crisis intervention skills

Education Required

Masters in Social Work (MSW)

Registration/Certification

Licensed Independent Clinical Social Work (LICSW)
