The Nurse shall administer, coordinate, and monitor all health care
services for residential participants. In doing so, he/she shall
establish and maintain appropriate relationships with families, other
health care providers, pharmacists and residential providers. ESSENTIAL
DUTIES 1. Participate in all pre-admission evaluation studies. 2.
Participate in the periodic review/re-evaluation of the type, extent,
and quality of services and programming. 3. Insure that each participant
possesses a complete medical case history. 4. Participate in all
interdisciplinary team reviews and attend all related meetings as
assigned by Senior Vice President or designee. 5. Recommend lab tests,
x-rays, EEGs and other diagnostic tests as needed. Note test results. 6.
Insure medical records and procedures are in compliance with local,
state, and federal regulations. 7. Participate in the referral to
appropriate community resources. 8. Coordinate medical services with
residential nursing staff. 9. Confer with other health care
professionals and team members as needed (i.e., dentists, audiologists,
therapists, physical, occupational, speech, program coordinators, etc.)
re: participant progress and treatment plan/programming. 10. Communicate
participants? health needs and plan of care to Day Habilitation staff,
as appropriate. 11. Control communicable diseases and infections by (a)
identification and assessment; (b) reporting to proper authorities; (c)
initiation of appropriate protective/preventative measures; (d)
documentation. 12. Respond to emergency situations as needed and
document properly. 13. Update re-certification orders annually. 14.
Document participant immunizations and update as needed. 15. Provide
documentation re: participant?s responses to medical
interventions/nursing notes. 16. As applicable, provide participant
instruction in such areas as safety, first aid, health and nutrition,
personal hygiene, and exercise. 17. Provide trainings to Agency
employees per MAP policies. 18. Communicate and coordinate all relevant
information to the nurses and staff at both the day program and at the
participant?s residence (all shifts). 19. Act as a resource for the
residential program as needed. 20. Coordinate and track all appointments
for residents. 21. Provide initial MAP training and recertification
testing to Agency staff as needed, with RN licensure status only. 22.
Additional duties as assigned. LPN (ABI & RES): schedules vary, all full
time, $35/hr. Locations: Wareham, Rehoboth, Marion, Taunton, Plymouth
and North Dighton.
