Certified Nursing Assistant Direct Care Support are responsible for
providing an array of services to enrich the lives of people with
developmental disabilities. Various levels of support are provided in a
number of areas including, but not limited to, personal care, life
routines, finances, health care and clinical services, recreational and
social interests, developing relationships and connecting to the
community. ESSENTIAL DUTIES 1. Enhance individuals lives by monitoring
needs and offering personalized assistance and supports with activities
of daily living to improve quality of life. 2. Assess and report any
issues to other healthcare personnel. 3. Supervise, support, and
maintain the safety of assigned individuals. 4. Provide support,
training and assistance to individuals as assigned by supervisor. These
areas may include personal hygiene, daily living skills, recreation,
and/or other skills as determined by the individual?s ISP and as needed
in life routines. 5. Participate as a team member in the ISP process and
in person-centered planning as applicable. 6. Implement support plans
throughout the day, including behavioral and therapeutic requirements.
7. Provide case management responsibilities and/or service documentation
requirements as assigned and defined by the supervisor. 8. Attend
meetings, as required, to facilitate participant progress, Agency growth
or general communication among staff and for other agencies. 9. Provide
training, support and assistance as necessary to aid people in adapting
to and participating in community environments. 10. Attend all required
trainings and maintain required certifications. 11. Attend work on a
regular, consistent basis. 12. Provide or facilitate transportation
arrangements reinforcing the use of natural supports when possible. 13.
Perform all duties in conformance to, or in recognition of existing
Agency philosophy, policies and procedures. 14. Assist individuals with
medical appointments as necessary. 15. Administer and document
medication in accordance with DDS regulations. 16. Ensure all required
documentation is completed which includes but is not limited to progress
notes; program data; medication; cleaning checklist; daily log/staff
communication; finances; and time slips. 17. Other duties as assigned.
If they have a CNA license, they will start at $20/hr and once MAP
certified they go up to $21/hr. Locations: Wareham, North Dighton,
Taunton, Rehoboth, Norton, Raynham, and Lakeville.
