These nurses work collaboratively with the patient, family, patient care team, health insurer and healthcare providers in the community to develop an individualized discharge plan. The Case Manager works as a member of the health care team in coordinating patient care services across the continuum, while promoting and monitoring the effective use of resources. The Case Manager demonstrates clinical expertise in the development of discharge plans, applies utilization review criteria to determine medical necessity and advocates for the patient as necessary, documents and collaborates with clinical and non-clinical staff, insurance case managers, referral representatives and Director. The Case Manager functions as a role model, educator, consultant and change agent in the area of care coordination as well as, participates in quality improvement initiatives, both departmental and hospital-wide.Qualifications:
Education:Graduate of an NLN accredited School of Nursing, Bachelor degree in nursing or a related field
Current licensure as a registered nurse in the State of Massachusetts
Minimum of 5 years nursing experience preferably in an acute care or managed care setting
2 years prior experience in Case Management, preferred (Discharge Planning and Utilization Review
Current knowledge of federal, state, and regulatory requirements guiding case management
Working knowledge of community and support services
Strong interpersonal and communication skills; ability to manage, teach, negotiate and collaborate with internal and external resources, patients and families
