About the Role:As a CARE Case Manager in our Consumer Support Services industry, you will be responsible for providing comprehensive case management services to individuals and families in need. You will work closely with clients to assess their needs, develop care plans, and connect them with appropriate resources and services. Your ultimate goal will be to help clients achieve greater independence, self-sufficiency, and overall well-being. This is a challenging and rewarding role that requires a high level of professionalism, empathy, and attention to detail.Minimum Qualifications:

Bachelor's degree in social work, psychology, or related field

2+ years of experience in case management or related field

Strong communication, organizational, and problem-solving skills

Ability to work independently and as part of a team

Knowledge of community resources and services

Preferred Qualifications:

Master's degree in social work, psychology, or related field

Experience working with diverse populations and/or in a non-profit setting

Bilingual in English and Spanish

Certification in case management or related field

Responsibilities:

Conduct comprehensive assessments of clients' needs and develop individualized care plans

Coordinate and monitor the delivery of services and resources to clients

Maintain accurate and up-to-date case records and documentation

Collaborate with other professionals and agencies to ensure clients receive the best possible care

Provide crisis intervention and support as needed

Skills:In this role, you will use your strong communication and interpersonal skills to build relationships with clients and collaborate with other professionals. Your organizational and problem-solving skills will be essential in developing and implementing effective care plans. You will also need to be proficient in using technology and software to maintain accurate case records and documentation.
