GREENFIELD Job Description At Pioneer Valley Hospice & Palliative Care,
located in Western, MA, our mission is to support and care for those
living with an advanced illness so they may live as fully and
comfortably as possible. We are a member of Berkshire Healthcare
Systems. Our nurses, social workers, hospice aides, spiritual
counselors, bereavement counselors, and volunteers will provide
information, resources, and expert medical care to support our patients,
their families, and caregivers with caring hearts and hands. The
Bereavement Coordinator will be responsible for planning, developing and
implementing the bereavement program, to include facilitating support
groups, community education, and other bereavement services in addition
to providing direct support to bereaved family members of hospice
patients, community members and to the interdisciplinary team Annually
plan, develop, and implement the agency's bereavement support services
and community bereavement services including remembrance ceremonies,
holiday coping program, grief support groups in community and contacted
facilities, etc.) Complete bereavement coping assessment of primary
caregivers following death of patient. Develop and implement a plan of
care for primary caregiver. Assess and provide supportive interventions
to high risk/special needs grievers. Develop, coordinate and implement
programs of grief support services for children, teens, and adults. This
includes mailings at appropriate times. Facilitate grief support groups
with populations that can include: loss of partner, general loss, loss
of adult child, child & adolescent grief, and others as assessed
community need is identified. Grief education and community outreach may
include school systems, councils on aging, and long-term care
facilities. Attend weekly Interdisciplinary Team meeting. Identify high
risk grief issues in patients/families. Follow up with supportive visits
as determined by IDT. Serve as consultant to the hospice team on grief
issues, needs and interventions, at IDT meetings or as needed. Meet
individual and program expectations and goals as identified through
performance appraisals, the annual plan, or by supervisor. Orient new
employees and others (volunteers, visitors, students, etc.) to
bereavement support services in hospice and provide an additional
general volunteer training per year. Assist in training and supervision
of Bereavement Support Volunteers. Participate in agency Quality
Improvement process. Demonstrate continuous growth in skills and
knowledge to the highest standard of practice. BC with Social Worker
credentials: will participate as SW backup (visits/admissions) as
needed. BC with Spiritual Care credentials: will participate in SC
backup (visits/admissions) as needed Hourly commitment per week for this
position based on credentials and professional experience. Other duties
as assigned by the Clinical Director and or the Executive Director which
may include messaging, marketing, fundraising, community outreach, and
community work. Job Requirements Education and Training: Masters degree
in social work, counseling, divinity, theology, pastoral counseling
Training in impact of severe illness, death and bereavement of
individuals/family systems desired. License, Certification &
Registration: Licensed by the state of Massachusetts as a clinical
social worker, marriage, family and child counselor, licensed mental
health counselor or psychologist required. OR Evidence of 2 Clinical
Pastoral Education units required. Possesses and maintains current CPR
Certification. Valid driver's license
