Position: Awake Overnight Reports to: Residential Program ManagerLocation: Sherwood Ave.Hours: Full-time 40 hrs. Sunday-Wednesday 11pm-9amTriangle’s Expectation of Employee:

Essential Duties and Responsibilities:

Provide direct care coverage, responding to individual issues, between 11:00p.m. and 8: 00a.m. on scheduled nights/mornings and as required by Program Manager.

Support individuals as needed or directed, in the areas of personal care, meal preparation and eating, problem solving, medical care, socialization and leisure activities, interpersonal skills, household tasks, money management, activities of daily living, asserting their human and civil rights.

Make calls at designated intervals to confirm awake status.

Perform cleaning tasks in all areas of the residence, including bathrooms, Kitchen, Living Room, Family Room, Laundry Area

Wash and fold residents’ laundry (in cooperation with 3-11 staff).

Responsible for calling or coming in shortly before 11:00p.m. for an update of program status.

Know emergency procedures and basic emergency facts regarding all individual living at the location where coverage is being provided.

Know and adhere to all safety policies and emergency procedures. Know basic emergency facts regarding all individual living at the residence.

If certified to dispense medications as prescribed, report any medication occurrences to Program Manager, complete necessary forms and follow applicable procedures.

If certified, dispenses medications as prescribed.

Complete any necessary documentation, including (but not limited to) Communication Log, Case Log, ROADs Charts, Daily Progress Notes, ISP Assessments, Support Strategies, Incident Reports, Medication and Treatment Charts, Medication Occurrence Reports, Communication Logs between day/residential supports, Money Counts, etc.

Attend and participate in supervision and staff meetings as scheduled by the Program Manager.

Attend and participate in bi-weekly staff meetings.

Attend trainings as directed by the Program Manager.

Maintain TRIANGLE property in good and sanitary condition. Keep all areas of the residence clean, with assistance from residents, as appropriate to the setting.

Treat all information about persons supported as confidential.

Qualifications

High school diploma or general education degree (GED)

1 year of related experience and/or training, or equivalent combination of education and experience.

CPR/First Aid Certificate

Valid MA license, availability of vehicle

Language Skills

Ability to read, analyze, and interpret documents. Ability to respond to common inquiries or complaints from regulatory agencies, or family members.
