Growing together.As a Field-based Community Health Worker (CHW), you will act in a liaison role with Medicaid members to ensure appropriate care is accessed, as well as to provide home and social assessments and member education. CHW’s work in a team-based structure and spend approximately 75% of their time in the community engaging directly with members. Interest in learning to work with medically complex patients who may be experiencing significant addiction and/or behavioral health conditions is important for this role.If you reside within Plymouth, MA market, you will have the flexibility to work remotely as you take on some tough challenges.Primary Responsibilities:

Create a positive experience and relationship with the member

Help member set goals and develop a care plan to achieve those goals

Proactively engage the member to manage their own health and healthcare

Support the member to improve their wellbeing by staying out of the hospital, and attend regular visits to their primary physician

As needed, help the member engage with mental health and substance use treatment

Support the member to ensure access to member care needs, including SDOH needs

Provide member education on community resources and benefits

Conduct post discharge activities from hospital facility and support connection to social services

Support member to engage in work or volunteer activities, if desired, and develop solider social supports through deeper connections with friends, family, and their community

Engage members face-to-face and/or telephonically to complete a comprehensive needs assessment

Develop and implement individualized, person-centered care plans inclusive of goals, opportunities and interventions aligned with a person’s readiness to change to support the best health and quality of life outcomes by meeting the member where they are

High School Diploma / GED (or higher)

1+ years of experience with MS Office, including Word, Excel, and Outlook

Able to comply with UHG Recruiting Policy and have a designated workspace with access to high-speed internet

Reside within the local community for 2+ years, with knowledge of culture and values of community and familiarity with the resources available in the community

Reside within a commutable distance of one of the following Counties: Bristol, Plymouth, Suffolk

Reliable transportation and the ability to travel within assigned territory to meet with members and providers, locally, approximately 75% of the time (mileage is reimbursed)

Preferred Qualifications:

Bachelor’s Degree (or higher) in a health-related field

CHW Accreditation

Field based experience

Care management experience

Experience working in Managed Care

Knowledge and/or experience with behavioral health or substance use disorders

Knowledge of Medicaid and/or Medicare population

Physical Requirements

Ability to transition from office to field locations multiple times per day

Ability to navigate multiple locations/terrains to visit employees, members and/or providers

Ability to transport equipment to and from field locations needed for visits (ex. laptop, etc.)

Ability to remain stationary for long periods of time to complete computer or tablet work duties
