Overview: We are seeking a dedicated Case Manager to join our team in Lynn. The Case Manager will play a vital role in providing direct support and coordination for residents participating in our program, with a focus on helping them achieve their goals. This full-time position offers a rewarding opportunity to make a positive impact in the lives of young adults and their families.

Position Details:

Role: Full-time, Monday through Friday, 9AM to 5PM.

Responsibilities: The Case Manager will be responsible for maintaining and updating residents' records, offering direct support, and coordinating services in accordance with program policies and procedures.

Essential Functions:

Work with young adults and their families, providing support in transitional care and aftercare outreach.

Utilize personal vehicle for aftercare outreach when Centerboard vehicle is unavailable.

Demonstrate flexibility in scheduling to meet client and program needs.

Implement principles of Trauma-Informed Care.

Coordinate intakes and complete associated paperwork promptly.

Collaborate with community agencies to coordinate services for clients.

Facilitate treatment team reviews and update treatment plans regularly.

Conduct assessments and maintain accurate records in compliance with regulations.

Provide crisis intervention and support clients with coping skills.

Support residents in medical, dental, and clinical appointments.

Develop and facilitate program groups to support resident needs.

Act as a positive role model and leader for clients and colleagues.

Participate in On Call rotation and document calls as required.

Conduct in-house trainings for staff on various topics.

Assist with discharge and transition planning for residents.

Conduct interviews and intakes with new residents.

Monitor resident budgets and assist with financial management.

Contribute to Continuous Quality Improvement efforts.

Perform other tasks and assignments as requested.

Competencies and Qualifications:

Education: Bachelor's Degree preferred, but not required. Relevant experience in social work, case management, or related fields may be considered.

Experience: Strong communication and organizational skills are essential. Previous experience in similar roles is advantageous.

Driver's License: Must possess a valid driver's license and the ability to drive 7-passenger vans. Access to and use of a personal vehicle may be required.
