Harbor Health is seeking a Medical Scheduler to join our team at the Elder Service Plan (ESP).
Home base will be be in our Brockton location (479 Torrey Street).

Role:
The Medical Scheduler schedules all medical specialty appointments for Elder Service Plan participants, coordinating with participants and family as needed. The Medical Scheduler is responsible for follow up, documentation, scheduling and coordinating transportation, cancelling/rescheduling and all other support needed to facilitate the efficient flow of medical specialty appointments for all participants.
Responsibilities:
Schedules medical appointments with medical specialists (both internal and external), coordinating with the participant and/or family as needed.
Collects documentation and forms necessary for the appointment in coordination with the Nurse Manager.
Documents schedule in participant’s medical record and the required work-flow documents in NextGen EMR and Excel.
Coordinates transportation and escorts for scheduled appointments.
Documents scheduled appointments in EMR and documents completion, reason for cancellation and re-scheduling of appointments as needed.
Notifies assisted living facilities, family members, participants and IDT members as needed of scheduled appointments.
Distributes weekly referral reports to IDT members, creates and mails podiatry appointment notices to participants.
Attends weekly appointment meeting with Nurse Case Managers, Center Nurse Manager and Transportation coordinator.
Must be able to utilize critical thinking skills to problem solve throughout the day.
Assists with reception and phone coverage as needed.
Requirements:
High School diploma or GED
Two years of experience working in a health care environment or Associates degree and one year of healthcare experience or Bachelor’s degree in related field
One year of experience working with and elderly population required
Intermediate skill level with Microsoft Word, Outlook, and Excel
Experience with CRM software highly desired
Proven ability to resolve problems creatively
Excellent verbal and written communication skills and organizational skills
Ability to work independently and as a member of a Team
Successful completion of a Medical Terminology course preferred
