LPN Outpatient Nurse - Community Behavioral Health Center (CBHC)

We have an exciting opportunity to join Riverside Community Care as a Nurse for our new Community Behavioral Health Center (CBHC) Component!

The Community Behavioral Health Center is a new provider model serving as an entry point for timely, flexible, evidence-based treatment for mental health, substance use, and co-occurring disorders for MassHealth members of all ages. The Riverside centers in Milford and Norwood will now serve as two of 25 different CBHCs across the Commonwealth, with integrated teams of nurses, clinicians, peers, and case coordinators providing routine and urgent outpatient care, as well as 24/7 community-based crisis intervention as an alternative to hospital emergency departments.

In this new vital role, the nurse is an integral part of the CBHC interdisciplinary treatment team, including Nurse Practitioners, prescribers, clinicians, case managers, peer/recovery coaches, and medical assistants. The Nurse is responsible for providing direct care and medical care coordination for adolescents and adults.

The Nurse will complete regular medical screenings for all CBHC individuals as well as assessments, follow up on lab work and referrals or admissions, and consults to other team members. The Nurse will also be involved in the medicated assisted treatment (MAT) program, provide injectable antipsychotics, and support people who have complex medical concerns and are in need of nursing case management.

Schedule: Part-time 24hrs weekly: every Thursday and Friday 8 or 12 hour shifts available.

Rate: $35.02/hour

Required Skills

Excellent communication, interpersonal and organizational skills

Intermediate computer fluency, including Microsoft Office

Valid Driver’s License and regular access to a reliable vehicle

Required Experience

Must be a graduate of an accredited school of nursing.

Licensure as a LPN required; Licensure as a Registered Nurse preferred

Must be in good standing with the Board of Registration of Nursing

Minimum of 2 years’ nursing experience

Minimum of 1 year working in a behavioral health setting

Experience with addictions and Medication Assisted Treatment strongly preferred.
