Who We Are: Since 1985, BHCHP’s mission has been to ensure unconditionally equitable and dignified access to the highest quality health care for all individuals and families experiencing homelessness in greater Boston. Over 10,000 homeless individuals are cared for by Boston Health Care for the Homeless Program each year. Job Summary: The purpose of this position is to provide leadership and follow through for the team in discharge planning and social service needs. The case manager will provide expertise to the medical teams to which he/she is assigned in regards to timely discharge placement, assistance with social service needs (e.g., identification, birth certificate requests, transportation, referrals to outside agencies, etc.), and by connecting patients to appropriate treatment options in the community post-discharge. The case manager works collaboratively with all members of the clinical team. Hours: Per Diem/As needed: 8am-4:30pm, Monday-Friday Responsibilities: Primary Duties:

Works as part of the multidisciplinary team with the primary responsibility of discharge planning. Communicates daily with the team

Maintains appropriate documentation in the medical record so that the case management plan (including appropriate discharge placement) for each patient is easily accessible to the team and others

Provides resources to assist patients in breaking cycles of homelessness, addiction, and/or mental illness, secure appropriate discharge placement and, when possible, other appropriate alternatives to shelter living.

Works with the other team members to problem-solve and develop a plan when patients present with challenging behaviors

Connects with community agencies to secure services such as vocational rehabilitation, physical rehabilitation, mental health services, substance abuse services, etc.

Reports all discharge planning to the clinical team and Supervisor of Case Management to ensure clear communication regarding the discharge plan. Communicates patient discharge dates to the Admissions office

Actively contributes to discussion and provides updates to the team during Team Rounds

Builds on the discharge plan from week to week and demonstrates this process in team discussion and notes

Meets with supervisor as directed and participates in weekly case management team meeting

Secondary Duties:

Provides documentation for grants, projects, etc as directed by supervisor

Runs a weekly treatment group focusing on recovery awareness and substance abuse issues

Attends and participates in meetings as directed

Qualifications:

Previous case management or human service experience required

Excellent communication, documentation and organizational abilities

Bachelor's degree; BSW or social service degree preferred

Spanish speaking required

Ability to be flexible, multi-task and prioritize responsibilities

Ability to work in a fast-paced environment and handle stress in a positive manner

Demonstrated interest in working with underserved populations

Compensation and Benefits:

The compensation is $22.00 per hour
