The ideal candidate will work within a multidisciplinary team to provide physicals and medical care to existing clients who are receiving behavioral health services as well as care to new clients as they join the practice. They will be expected to conduct initial Physical Assessment and plan ongoing care for children and adults.They will be responsible to:Conduct ongoing and wellness examinations and pharmacological evaluationsPrescribe medications, teach clients about side effects and monitor response to prescribed medications Provide services to clients based on current status, risk, and as clinically indicatedSupport staff to provide recovery-oriented and trauma-informed servicesParticipate in staff meetings Provide crisis intervention services supporting staff as neededCommunicate with hospital ER attending  when clients access emergency servicesSupport discharge planning with hospital and ER staffConduct risk assessment, provide guidance and direction on risk managementUse e-prescribingComplete progress notes, significant events, referrals, and evaluations within expected timeframesUse Electronic Medical Record (EMR) and data reports from EMR to track treatment progress and individual client outcomesUse collaborative documentation procedures for assessment and individual sessions with clientsCommunicate with specialist and behavioral health team to deliver integrated treatment Work in collaboration with the client and significant others to support recovery goals
