Consultant Pharmacist - Baltimore, MDThe CommuniCare Family of Companies currently owns/manages numerous World-Class Nursing and Rehabilitation Centers, Specialty Care Centers and Assisted Living Communities throughout Ohio, Pennsylvania, Maryland, and West Virginia. Since 1984, CommuniCare Health Services has provided superior, comprehensive management services for the development and management of adult living communities.CommuniCare Health Services is looking for an experienced Clinical Consultant Pharmacist to fully integrate into its interdisciplinary teams. We are looking for a pharmacist that strives for excellence and enjoys collaborating with other healthcare professionals to provide comprehensive medication management services. The pharmacist will utilize both their clinical and analytical skills to achieve positive clinical outcomes for our residents. This unique opportunity will be based in our long-term care/post-acute care facilities in the Baltimore, MD area. Travel is required throughout the assigned region.As a clinical consultant pharmacist you will:

Provide your clinical and professional expertise on all aspects of pharmaceutical services

Perform monthly medication regimen and admission reviews

Effectively collaborate with the interdisciplinary team to provide pharmaceutical care

Lead the monthly Pharmacy & Therapeutics clinical team meeting

Challenge and grow your expertise beyond traditional regulatory requirements

Requirements:

Minimum one (1) year experience as a consultant pharmacist in LTC/PAC facilities

BS Pharmacy or PharmD degree

Certified Geriatric Pharmacist (CGP) certification, preferred

Valid pharmacist license in the State of Maryland

Exceptional organizational skills and the ability to work independently and collaboratively
