Clinical Counselor/Patient NavigatorJob DetailsJob Location342 Augusta Road - Winslow, ME Position TypeFull Time DescriptionThe Clinical Counselor/Patient Navigator supports individuals with an identified Substance Use Disorder. The Clinical Counselor/Patient Navigator will provide individual and/or group counseling related to substance use disorder. This position will work with the member receiving OHH services to collaborate with other health care, mental health, social services, and community providers to guide the member in accessing additional services and supports that will help the member in their recovery. For OHH members who are receiving coordinated case management services, the Clinical Counselor/Patient Navigator shall be the primary provider of care coordination, health promotion, individual and family support services, and referral to community and social support services. This position will be required to meet the requirements of a regular office schedule, working five days per week, Monday-Friday, between 8am and 5pm. Duties include and are not limited to: Clinical Counselor

Providing assessment and treatment of substance abuse disorders, including assessment of co- occurring disorders and referral to other services as appropriate.

Documenting each client’s comprehensive assessment, individualized treatment plan and all treatment plan reviews, discharge summaries and progress notes.

Completing Kepro continuing stay reviews as needed.

Maintaining appropriate licensure, including following all continuing education requirements in order to ensure that clients are receiving the best possible clinical services.

Adhering to all state regulations, policies and contract parameters.

Other duties as assigned

Patient Navigator

Developing an Individualized Service Plan (ISP) for each OHH member within 30 days of admission to the OHH and reviewing the plan every 90 days thereafter.

Completing annual paperwork for each OHH members within allotted timeframes.

Ensuring releases of information for new and existing OHH members are current.

Working closely with all members of the support team to ensure continuity of care

Developing and maintaining thorough knowledge of referral resources

Developing and maintaining up-to-date knowledge of the MaineCare application process and status of all clients on case load

Maintaining up-to-date knowledge through participation in continuing education as related to position

Developing and maintaining crisis plan

Maintaining timely funding authorizations and prevention of gaps in authorization

Informing Clinical Director and Quality Assurance of any client complaints and reportable client incidents (internal and state level)

Completing and forwarding, as appropriate, any incident report to the Clinical Director for review and approval and then to the appropriate state office

Providing excellent customer service by responding within the current business day to all forms of communication (i.e. Phone calls, voicemails, emails, etc.)

Establishing and maintaining professional boundaries with clients

Maintaining strict confidentiality of client and staff information

Attending and/or facilitating team meetings for clients

Attending all OHH team meetings, supervisions and educational opportunities as directed.

Meeting the billable expectation of 50% of hours worked weekly

Other duties as assigned

QualificationsPosition Requirements:

Must be 18 years of age or older

Proof of vehicle insurance at $50/$100K and a valid Maine driver's license

Intermediate level computer experience

A current, active certification as one of the following: CADC, LADC, LCSW, LMSW-CC, LCPC, LCPC-C, LMFT, or LMFT-C

60 hours of alcohol and drug education within the last 5 years, if not CADC or LADC certified
