Join us and together we will drive the future of health care.Location: This is a virtual position, but you must reside within 50 miles of an Elevance Health Pulse Point.Work schedule: Monday - Friday 2:30 pm - 11 pm EST.The Nurse Disease Management Lead is responsible for maintaining an active caseload of the most complex cases and for serving as a lead by assisting team members in the delivery of disease management programs and by providing guidance and assistance with clinical, program, and client-specific questions and situations.How you will make an Impact:

Conducts behavioral or clinical assessments to identify individual member knowledge, skills, and behavioral needs.

Identifies and/or coordinates specific health coaching plan needs to address objectives and goals identified during assessments.

Interfaces with providers and other health professionals to coordinate health coaching plans for the member.

Implements and/or coordinates coaching and/or care plans by educating members regarding clinical needs and facilitating referrals to health professionals for behavioral health needs.

Uses motivational interviewing to facilitate health behavior change.

Monitors and evaluates the effectiveness of interventions and/or health coaching plans and modifies them.

Directs members to facilities, community agencies, and appropriate providers/networks.

Refers members to catastrophic case management.

Monitors assigned telephone skill groups and work volumes to meet individual and team performance expectations as related to call volume, call length, and productivity.

Identifies potential obstacles to the attainment of service standards and reports possible solutions to the Manager.

Assists in the development of new programs and initiatives, leads cross-functional teams, and may represent disease management or business unit on enterprise initiatives.

Minimum Requirements

Requires AS in nursing and a minimum of 5 years of recent related clinical experience and a minimum of 2 years of experience in disease management; or any combination of education and experience, which would provide an equivalent background.

Current unrestricted RN license in applicable state(s) required.

Preferred Skills , Capabilities, and Experiences:

BS in nursing preferred.

A broad clinical knowledge base of disease management processes and motivational interviewing is strongly preferred.

The ability to negotiate and facilitate the resolution of treatment is strongly preferred.

For candidates working in person or remotely in the below locations, the salary range for this specific position is $83,840 to $150,912.Colorado Locations: California; Colorado; Hawaii Nevada; New York; Washington State; Jersey City, NJ.In addition to your salary, Elevance Health offers benefits such as a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401k contribution (all benefits are subject to eligibility requirements).
