With our combined resources CommonSpirit is committed to building healthy communities advocating for those who are poor and vulnerable and innovating how and where healing can happen both inside our hospitals and out in the community.ResponsibilitiesUnder the direction of the Cancer Center Director, the Nurse navigates cancer patients through the continuumof cancer care providing education, interpretation, emotional support, support group opportunities, etc. foroncology patients and their care givers. The nurse serves as the primary point of contact coordinating andcollaborating with multiple physician specialties for care planning and services across the continuum of careeffectively utilizing resources and multidisciplinary teams to identify and eliminate barriers to care in order toachieve an optimal clinical outcome for the patient and their caregivers. The nurse participates in CancerCenter tumor conferences, disease site steering committees, health fairs and education sessions supportingeducation and program development.ESSENTIAL KEY JOB RESPONSIBILITIES● Demonstrates the knowledge, skill and coordination to provide nursing care and guidance to the cancerpatient from first contact and throughout follow-up, serving as an essential link between patients and allother care providers.● Systematically and continually performs the functions of assessing, planning, implementing and evaluatingthe care according to NCCN guidelines, CHI Saint Joseph Health standards, and ONS standards of practice.● Facilitates the coordination of patient care services to assure excellence in patient care and patient flow bya) following the patient through the care continuum/experience, eliminating operational (i.e. scheduling,test results, etc) barriers to service; b) working closely with other health care disciplines to ensure timelyappointments, results reporting, financial need referrals, communication, patient care and follow-up.● Interfaces with other healthcare teams for appropriate referrals/services such as the social worker,pastoral care, dietician, dental hygienist, community services, financial counselor, pharmacy coordination,home care, cancer rehabilitation, clinical research nurses, survivorship nurse oncology, palliative care,hospice, smoking cessation, etc.● Initiates and documents patient teaching including family and significant others based on assessment ofneeds, including responding to patient request for information regarding the disease process, expectedJOB DESCRIPTIONPage 2 of 5 JOB DESCRIPTION MASTER TEMPLATEside effects of treatment and community resources. Documents the above appropriately as well ascommunicating teaching completed to the patients other health care providers as appropriate.● Tracks and communicates monthly referrals by disease site and distribution as requested.● Continually assess the patients treatment progress and educational needs, making adjustments to carebased on the assessment; consults with primary physician or designee and other members of the caredelivery team as needed; refer to appropriate physicians, NPs or programs when needed.● Collaboratively works with clinical research nurses, facilitating patient referrals for potential clinical trialcandidates, communicates patient care needs and schedule with the clinical trial nurse if patient acceptedinto a trial, negotiates patient care and teaching responsibilities with the clinical trial nurse when thepatients on a clinical trial.● Active participant on the multidisciplinary care team and conferences, providing specific patient careinformation pertinent to the patient, follows up on patient care recommendations following themultidisciplinary conferences, and when necessary, facilitates the multidisciplinary conference.● Facilitate/organize patient support groups as appropriate.● Serves as a resource for community educational events such as health fairs, screenings, support groupsand utilizes the resources of CHI Saint Joseph Health organization.● Coordinates pertinent patient information, notifying appropriate MDs from pathology, radiology, medicaloncology, radiation therapy, surgeons and research staff and follow up on all patient care.● Other duties as assigned by management.The job summary and responsibilities listed above are designed to indicate the general nature of the work performedwithin this job. Graduate from an accredited nursing program.

Required Licensure and Certifications:

RN:KY

BLS-CPR within 5 days

Required Minimum Knowledge, Skills,Abilities and Training:

Minimum of 5 years’ experience in inpatient or outpatientoncology, surgical services or related.
