Travels to senior communities and homes to provide support to our hospice clients and their families

Performs client assessments

Participates in interdisciplinary group meetings to discuss patient goals and care plans with other care providers on the team

The pay range for this position is $22.28 - $28.07 / hour. The starting pay rate will be based on years of experience.

Mileage Reimbursement at 58.5 cents per mile

Valid driver’s license, reliable transportation, valid insurance, and clean driving record

Ability to pass a pre-hire background check and drug screen

Exceptional interpersonal skills to deal effectively with clients, providers, family members and team members

Experience in hospice and working with families and individuals throughout the death and dying process

Graduate of accredited seminary or school or theology
