Apply today for our Registered Nurse RN Stroke Coordinator opportunity.Job Summary and QualificationsThis position involves the coordination of Stroke clinical management of all patients across the continuum. Serves as leader of the Stroke programs and provides education to all staff including the medical staff. Assists with community education for a better understanding of Stroke care.What will you do in this role:

Maintains and demonstrates clinical expertise in the area of Stroke standards of care.

Reviews best practices and shares information with all staff and medical staff. Reviews up-to-date evidence based literature and shares information with the staff and medical staff.

Collaboratively works with the medical staff and interdisciplinary teams to develop and implement clinical pathways, order sets and algorithms.

Collaboratively works with all staff and medical staff to ensure Stroke Core Measures are implemented timely.

Reviews Stroke polices annually. Revises based on up-to-date literature and change in standards of care.

Works closely with the PI Coordinators to ensure all measures are abstracted accurately and timely.

Responds to all Code Strokes to facilitate the completion of all requirements.

Chairs the Interdisciplinary Sepsis and Stroke Committees. Serves as the leader of Stroke process improvement initiatives.

Participates in monthly Stroke Capital division calls.

Monitors and performs data collection for all Stroke patients. Aggregates and analyzes data for identification of opportunities for improvement.

Attends Nursing Leadership meeting and provides Stroke updates.

Responsible for Stroke post discharge follow-up calls. May delegate to Stroke Abstractor.

Directs development and implementation of patient/family Stroke education.

Directs education for EMS on Stroke initiatives and provides feedback as needed for process improvement.

Provides orientation of the medical staff on Stroke measures and expectations.

Provides physician specific data on a routine basis

What qualifications will you need for this role:

Current Indiana nursing license or active compact state nursing license

Associates degree of nursing required. BSN preferred

3-5 years of hospital experience with Stroke experience preferred.

Current BLS certification required

Computer skills strongly preferred: Meditech, Word, Excel and PowerPoint
