Job Title: Case Manager – Sami Family Services

Location: Indianapolis, IN

Position Overview: We are seeking a dedicated and compassionate Case Manager to join our team at Sami Family Services. As a Case Manager, you will be with Department of Child Services and will play a crucial role in providing support and guidance to children and families involved with the child welfare system. This position requires strong interpersonal skills, attention to detail, and a commitment to advocating for the well-being of children.

Responsibilities:

Conduct assessments to determine the needs and strengths of children and families.

Develop and implement individualized case plans in collaboration with clients and other stakeholders.

Coordinate services and resources to address the needs identified in the case plan.

Monitor progress and ensure that services are delivered in a timely and appropriate manner.

Advocate for clients within the child welfare system and in the community.

Maintain accurate and up-to-date case records and documentation.

Participate in case conferences, court hearings, and other meetings as required.

Collaborate with other professionals, including social workers, therapists, and educators, to ensure a comprehensive approach to service delivery.

Qualifications:

Bachelor's degree in social work, psychology, counseling, or a related field required.

Previous experience working with children and families preferred.

Knowledge of child welfare policies and procedures is a plus.

Strong communication and interpersonal skills.

Ability to work effectively both independently and as part of a team.

Excellent organizational and time management skills.

Commitment to cultural competency and working with diverse populations.

Valid driver's license and reliable transportation.
