Case Management Program Specialist

Summary

Plan, implement, monitor and coordinate all case management activities for education, employment and supportive services.

This will consist of developing and maintaining case plans and files, providing feedback relating to program activities and updating case management activities.

Supports the Jobs Plus Program Manager’s vision and direction.

This position is a grant funded position.

All activities must support the Fort Wayne Housing Authority (“FWHA” or “Authority”) mission, strategic goals, and objectives.

Essential Duties and Responsibilities

The position duties and responsibilities listed below describe the general nature and scope of work. Other responsibilities, duties, and skills may be required and assigned, as needed.

- Participates and assists the Manager in grant application processes.

- Assist Manager in fundraising activities.

- Complies with data privacy laws ensuring confidentiality of participants’ information.

- Works with the Manager to achieve common program goals, ensuring commitment for the clients’ success.

- Continuously focused on engaging and interacting with clients via calls, meetings (in-person and virtual), and emails.

- Collaboration with all stakeholders/team members is essential in the success of this role.

- Maintains a continuum of education and employment related resources, in conjunction with community providers and program requirements.

- Ensure supportive services are provided to the target populations and makes appropriate referrals to grant partners and other community resources.

- Assists in the development of informational materials pertaining to program services and coordinates material distribution.

- Completes any special case assignments as instructed by the Manager and in coordination with community resources.

- Responds to inquiries, requests, and complaints, as necessary, and in a timely manner.

- Dedicated to working directly with a diverse population.

- Ability to cultivate and develop inclusive and equitable working relationships with employees, clients and community members.

- Supports and enhances a sense of belonging with employee, client and community member interactions.

- Work towards equity and our agency's commitment to diversity, equity, inclusion and belonging.

- Promotes FWHA brand awareness by adhering to FWHA brand standards and ensure that any communication regarding the public value of affordable housing programs offered by FWHA also addresses the many other public values served (fair housing, economic development, welfare-to-work, etc.)

- Performs other duties as assigned.

Education and/or Experience

- Associates Degree or Minimum of 2 years of experience in case management is required, or an equivalent combination of education and experience.

Strong organizational skills with the ability to manage multiple priorities effectively.

- Public speaking, listening, and written communication skills are required.

- Ability to work collaboratively in a team environment and build partnerships with diverse stakeholders.

- Commitment to promoting economic empowerment and social justice within underserved communities.

- Knowledge of community outreach strategies and marketing principles.

- Knowledge of HUD and organizational regulations, policies and procedures

- Experience with federally funded programs.

- Must possess a valid state issued driver’s license and be insurable under FWHA’s policies.
