Position Summary:

Works with the Marketing & Admissions team as well as the Treatment Team to coordinate admissions, maintain assessment outcomes data, and to provide follow-up contact to patients and referents following the assessment process as needed. Works with IIAR Assessment Specialist and Corporate Clinicians to ensure patient eligibility for treatment. Works with the program manager to complete assigned tasks related to quality assurance. Also works with the treatment team to ensure patients are appropriately referred from inpatient to outpatient services.

Qualifications:

EDUCATIONAL REQUIREMENTS H.S. Diploma/GED CERTIFICATION & LICENSURE REQUIREMENTS Certified Alcohol & Drug Abuse Counselor (CADC) within 3 years. EXPERIENCE REQUIREMENTS At least 2 years of relevant experience in addiction treatment or admissions required. SKILLS AND KNOWLEDGE Advanced reading, writing and oral skills. Ability to respond appropriately to customer/co-worker Interaction with a wide variety of people Maintain confidential information Ability to project a professional, friendly, helpful demeanor. Basic computer knowledge: Uses word processing, spreadsheet, e-mail application, and web browser. Comfortable within Windows OS and learning new applications. Ability to utilize an electronic medical record.

Essential Functions:

ENSURES PATIENTS RECEIVE COMPREHENSIVE SERVICES FROM INITIAL CONTACT THROUGH ADMISSION- Works with the utilization reviewer, secretaries, and other team members to provide information, coordinate referrals & phone assessments, and aid patients and referents through the admission process. - Works with the Patient Financial Services Department to ensure patients meet financial eligibility for treatment services.- Coordinates with the patient, referent, etc. any needed paperwork, to be reviewed by the Marketing and Admissions Manager and Treatment team to determine appropriateness for services.- Conducts follow-up calls to individuals who are assessed but do not admit to services and conducts follow-ups with referral sources as appropriate.- Is responsible for maintaining updated records of admissions and outcomes.- Documents referent outcomes in EPIC and coordinates outpatient referrals with the patient's assigned counselor. - Works with other departments within TrilliumPlace and Carle Health related to referrals and access to treatment services.- Works with marketing to provide service awareness to outside referral sources/other treatment centers and providers. - Provides discharge planning for patients, in coordination with clinicians. Locates intensive outpatient and outpatient services, individual counseling services, long term treatment facilities, assisted living, and other outpatient services as needed.

EVALUATES PATIENT. PRIORITIZES CARE ACCORDING TO ACUITY AND DOCUMENTS APPROPRIATELY- Contacts the referral sources, with appropriate releases obtained, in order to initiate continuity of care through the risk classification and/or admission/treatment process. Coordinates with referents and treatment team to ensure continuity of care through all service levels.- Provides that all patient contact and interaction is based upon inherent dignity, individuality, culture and beliefs of each patient. Patient care is provided in a compassionate, humane, and non-enabling manner.- Provides patient care with attention/knowledge of age specific care.- Makes appropriate referrals as necessary and facilitates the referral process for patients as appropriate.- Ensures all assessments are scheduled and covered in a timely and appropriate manner. Works well with clerical staff & supervisory staff to ensure coverage of evaluation schedule.- Provides follow up for no call/no show and patients who disengage in treatment services- Completes weekly quality assurance checks by auditing patient electronic medical records to ensure compliance with policies related to entering documentation.- Completes the monthly dashboard and reports results to the ARC manager and Trillium Place Director of Substance Abuse and Addiction Treatment Services. - Participates in the QA committee. - Oversees all service inquiry calls and follows up as needed.- Oversees data and outcomes related to assessments, conversion rates, and services inquiry calls. Ensures all data is logged appropriately.

PROVIDES PATIENT Education Gives lectures/education on assigned topics to patients/staff/community as needed and as assigned.
