Work directly with supervising Registered Nurse or Therapist.
Personal Care such as bathing, dressing, grooming, skin care, and toileting for patients in their home.
Follow HHA Care Plan set forth by supervising nurse.
Ability to document visits electronically and in a timely manner per agency policies.
Communicate any changes with the patient to the Nurse or Physical Therapist.
Responsibilities may vary based on patient's needs.
Be an advocate for your patients and their families as you provide care to allow them to maintain a safe quality of life.
HHA Qualifications:
Licensed as a Certified Nursing Assistant in the state of Illinois in good standings
Minimum 1 year of home health, caregiving experience, hospital, SNF, or ALF experience.
Valid Driver's License and reliable vehicle with current insurance.
Demonstrated ability to work as a team with supervising RN and interdisciplinary team.
Strong communication and interpersonal skills.
Comfortable with technology and must have a smart phone or mobile device.
Current CPR certification.
Strong communication and interpersonal skills.
Great with customer service.
