A New Age Human Services (ANAHSC) in Chicago, IL is looking for Direct Support Professional (DSP) to join our team. We are located on 5334 W Wrightwood Ave Chicago il. Our ideal candidate is a self-starter, motivated, and hard-working. We have flexible scheduling available Saturday and Sunday. we have evening and overnight shifts during the week. looking for part time only at the moment between 25 to 30hrs a weekResponsibilities· Supervise and assist individuals in activities of daily living by Providing personal hygiene assisting with baths, toileting, shaving, tooth care and personal grooming.· Implement active treatment program and document individual's progress.· Assist with completion of the individual’s assessments.· Document supports offered as needed by completing reports, forms and records· Participate in meetings and staffing as requested.· Clean facility in order to maintain a safe homelike environment.· Adhere to professional standards and Advance policies and procedures.· Perform other duties as requested by a supervisor.Qualifications · Education: High School Graduate or GED Substitution for Education· Maintain a valid Driver’s License· Demonstrate functional 8th Grade literacy through standardized exam· Obtain/maintain standing as an Authorized Qualified DSP's to pass medication· Complete DSP training in accordance with state regulations, and maintain status on the Health Care Worker Registry (HCWR)Benefits Flexible schedules health insurance dental /vision insurance paid time off We are looking forward to receiving your application.
