Case Manager (bloomingdale)

ALDEN VILLAGE HEALTH FACILITY is seeking a FULL-TIME CASE MANAGER. The Case Manager will integrate, coordinate, and monitor Active Treatment Programs for approximately thirty (30) residents in accordance with current Federal, State, and local standards, guidelines and regulations, facility policies, and as may be directed by the ADON/DON. The ideal candidate must have at least one (1) year of experience working directly with persons with mental retardation or other development disabilities; preferably a Direct Support Person. The ideal candidate must possess the ability to make independent decisions when circumstances warrant such actions. The ideal candidate must possess the ability to deal tactfully with resident, family members, visitors, personnel. The ideal candidate must have patience, tact, a cheerful disposition, and enthusiasm!
