Care Management Navigator

Position Summary: The Patient Care Navigator is responsible for assisting nurses and social workers to provide outreach efforts to a group of patients from a patient list. The Patient Care Navigator will share knowledge and education to empower patients and improve their ability to navigate services and systems. This role will utilize the established model of care management. The individual who holds this position exemplifies the Rush Copley mission, vision and values and acts in accordance with Rush Copley policies and procedures, including complying with all Rush Copley Medical Center Customer Service Standards.Position Requirements:· High school diploma or equivalent required. Associates degree preferred.· Must have excellent oral communications in order to solicit information and communicate effectively with stakeholders.· Knowledge of Microsoft Office.· Prefer knowledge of medical terminology.· Ability to function independently, to organize, and be self-motivated.Rush is an equal opportunity employer.
