Title: Community Care Team (CCT) + Behavioral Health Home (BHH) Nurse Care Navigator - Blended Role! A Behavioral Health Home Organization (BHHO) is a community- based mental health organization, that is licensed in the state of Maine, has been approved by MaineCare to provide Section 92 services for members eligible for such services.Andwell’s Behavioral Health Homes team welcomes, engages and serves individuals with co-occurring mental health and substance use. Case management services are delivered to adults and children, utilizing a team-based model of care. We aim to improve access to services while ensuring care planning is member driven and inclusive of natural supports as appropriate. What You’ll Do:
Collaborate with and coordinate community networks/teams and community-based support and service providers who share resources to ensure patients can access all the available supports the community has to offer.
Establish a supportive and connected community to improve the health of the patient, reduce unnecessary re-hospitalization and impact on health care services.
Primary care consultation and psychiatric care consultation, other services as necessary, pursuant to the Plan of Care.
What You’ll Bring:
Active RN license in the State of Maine required.
One year recent experience in the behavioral health setting preferred.
Current CPR card required.
