Position Title: Care Coordinator

Job Status: Full Time, Non-Exempt Salaried

Reports To: Executive Director

POSITION SUMMARY:

The Care Coordinator provides a range of supportive services and referrals to family members to ensure the safety of children and the well-being of family members. Responsibilities include identifying psycho-educational counseling services for children, teens, and adults in individual and group settings; providing a range of child building and parental life skills information and activities; coordinating, negotiating, procuring and managing clients’ service plan meetings, family case conferences and subsequent activities related to the case. Essential job functions include:

1) Conducts intake process with families

2) Performs Child Safety and Family Assessments

3) Completes Family Service Plan

4) Evaluates family dynamics and service needs

5) Coordinates all aspects of client services.

6) Provides a range of child building and parental life skill information activities

7) Assesses the need for services and forms links with appropriate resources

8) Attends meetings scheduled for families that are given resources

9) Logs and maintains all required client information and documents into database

10) Completes housing applications, and enters data into HMIS for homeless families

11) Relates with clients, public and co-workers in a mature and professional manner

12) Performs all duties with a high degree of accuracy, proficiency, and confidentiality

13) Periodically attend events and provide an information booth, on evenings and weekends

14) Other duties as assigned
