We are looking for a part time or full time Island wide Case Manager/Social Service Specialist to join our well established agency. We coordinate in-home care services for our senior community. We receive referrals and our team of qualified Case Managers do brief home visits, phone interviews and assessments for services such as personal care, homemaker, adult day care, etc. with a professional caregiver agency. We have the required PPE available and always put our staff/employees safety first. Work schedule Monday - Friday.
