The community access direct support staff will assist individuals while in the community and are required to follow the policies, procedures and standards of care while performing the various responsibilities:Executing the tasks and goals of the ISP.Executing planned activity calendar effectively.Supporting the individual with increasing their independence in their home and community.Attending and contributing to team meetings.Assisting in transporting the individuals to various community activities in accordance to the community activity schedule.Assisting with health and wellness related tasks (if required by the ISP). Scheduling and planning meaningful community activities with individuals.Providing advice and guidance to assist with decision-making and serving as an advocate to encourage the individual. Documenting all activities performed by the individual on a daily basis. Completing documentation and goal tracking in a timely manner.Working as a team with other employees to provide the best quality of support for the individual.Utilizing proper behavior support training and crisis prevention intervention techniques when necessary.Working well in a self supervisory setting, which includes taking initiative when necessary and engaging with individuals.
