Performs receptionist, telephone and clerical duties to support nursing staff in the delivery of care and services to unit residents. Maintains medical records for each resident that are complete, readily accessible and systematically organized. Interacts with residents, staff, consultants, visitors and others in a manner that facilitates communication and presents appropriate image of the facility.KEY RESPONSIBILITIES:

Greets visitors, staff and residents promptly in pleasant, courteous manner. Provides accurate information. Makes prompt efforts to answer questions and to resolve problems within scope of Unit Clerk responsibility or notify appropriate facility representative. Promptly notifies Unit Charge Nurse or Unit Manager of all resident and/or family concerns, grievances, and complaints

Answers the telephone promptly, courteously according to facility protocol. Takes messages accurately and promptly notifies recipient

Transcribes physician orders promptly and accurately. Mails written copy of all telephone and verbal orders taken on shift to physician for signature. Completes mailing before end of shift.

Schedules resident appointments within and outside the facility as directed by the Charge Nurse.

Maintains a clean and organize work space in the nurses’ station

Delivers messages and mail to residents promptly within 24 hours of delivery to the facility.

Assists residents with use of telephone while providing privacy for the resident’s calls.

Completes and updates daily unit records, maintains current unit copies. Routes documents such as census sheets to appropriate department according to facility policy.

Notifies departments, orally and in writing, when there is a change in unit census or resident census. Forwards written notices and records

Assumes clerical responsibility for updating resident clinical records, such as plans of care and conference notes, according facility policy.

Completes necessary transfer paperwork and ensures that all required records accompany residents who are transferred, discharged or scheduled for outside appointments. Inspects closed records for completeness, notifies Charge Nurse of missing information and forwards records to Medical Records when complete

Maintains unit inventory of equipment, supplies, and forms at levels sufficient to meet needs of unit Notifies appropriate facility staff member to recorder items before supplies are exhausted.

Maintains and updates current resource materials on unit, including policy and procedures, OSHA worker safety materials, exposure control plan for blood borne pathogens and other relevant materials as directed by Unit Manager.

Assists Unit Manager to prepare for long term care survey. Attends survey training and interacts with state surveyors as instructed by supervisor.

Reports housekeeping or maintenance problems to appropriate department for corrective action.

Communicates with Charge Nurse throughout shift to coordinate efforts and to meet resident and staff needs.
