Employment Type:Full timeShift:Day ShiftDescription:Position PurposeMercy Care Atlanta is seeking a Housing Manager to lead and develop housing programs that support patients with complex medical and social needs. This role will work with internal teams and external partners to implement best practices that improve housing stability and medical support for individuals experiencing homelessness.What You Will Do
Transitional Housing Strategy:
Design eligibility criteria and processes for Mercy Care’s ten transitional housing units.
Oversee daily operations to ensure a safe, comfortable, and effective housing environment.
Develop evaluation metrics to assess program success and provide progress updates to leadership.
Recuperative Care Innovation:
Conduct research on national recuperative care trends and make strategic recommendations for enhancements.
Supervise daily operations of the recuperative care program, ensuring best practices and efficient referrals.
Cultivate and maintain relationships with internal and external referral partners to improve service delivery.
Implement evaluation metrics to track program effectiveness and provide insights for continuous improvement.
Support for Complex Patient Populations in Permanent Supportive Housing:
Provide strategic guidance and training to staff working with complex patient populations.
Develop and implement best practices to enhance service delivery for patients in permanent supportive housing.
Establish and monitor evaluation metrics to assess program performance and report actionable insights to leadership.
Minimum Qualifications
Master’s degree in social work, psychology, human services, or a related field.
At least seven years of experience in human services, with five years in program development and supervisory roles serving disadvantaged populations.
Experience with multidisciplinary teams, grant writing and management, and program evaluation.
Working knowledge of community resources and system navigation for Mercy Care patients.
Valid Georgia driver’s license may be required.
