Work is performed under the supervision of the Public Health Services Manager F - SES. This position provides planning, monitoring, evaluation and consultation in the HIV/AIDS program. Function as the Quality Improvement/Hospital Coordinator for the HIV/AIDS Program. Primary responsibilities include development, implementation, monitoring and/or evaluation of FDOH in Palm Beach County Health Department HIV/AIDS Program’s quality improvement plan and consultation with local hospitals, as needed. This position requires independent judgment, initiative, applying general knowledge of business practices and skill in applying instructions to accomplish various job functions.This position spends the majority of the time engaged in supervisory responsibilities. This includes communicating, motivating, training, and evaluating employees in addition to planning and directing employees’ work. The incumbent has the authority to hire, transfer, suspend, lay off, recall, promote, discharge, assign, reward, or discipline subordinate employees or effectively recommend such action. The incumbent will also be required to develop performance expectations, meet with staff regularly to discuss performance, complete performance reviews, maintain vacation and work schedules, ensuring proper coverage, and complete timesheets.Supervise HIV perinatal linkage staff, Early Intervention Services staff, quality management/improvement support staff and the supervisor of the on-site case managers consultants and clinic peer mentors. This position is designated as a managerial position and is required to maintain confidential information in accordance with the Florida Department of Health (FDOH) Palm Beach County Health Department (CHD) Information Security Policy, Protocols and Procedures. This position may view, update, or release information. Incumbent will have access to the following confidential data sets: Clinical Information, HIV/AIDS Case Reporting and Surveillance, Administrative Data Set and Outreach. This position has access to the Electronic Health Record (EHR) for the purpose of training staff countywide who are responsible for entering data in the EHR and for quality improvement monitoring. Have access to the Health Management System (HMS) insurance option for the purpose of training case management staff and HIV contracted providers and meeting the requirements of our Ryan White billing process. This position will train case management staff/contracted providers on performing eligibility look up on the HealthCare District Database and Change Health Care (CHC) System in order to research clients’ eligibility determination. This position is the FDOH in Palm Beach County System Owner of the Ryan White CAREWare System. Have access to the Ryan White CAREWare and Provide Enterprise databases for the purpose of training staff countywide, entering data, quality improvement monitoring and HRSA funding requirement reports. Responsible for requesting CHC access for the HIV program as well as training new HIV program staff on how to use this system. This position is the FDOH in Palm Beach County System Owner of Access to Florida and will train the HIV case managers and the refugee case manager to use the online Medicaid system to assist potentially eligible Medicaid clients. Have access to the Surveillance Tools & Reporting System (STARS) for the purpose of verifying EIS staff’s performance in meeting the outcomes of the EIS program. Utilize DHSMV/DAVID computer application only to locate demographics of clients/patients who have a positive laboratory result and/or who have been exposed to someone with a positive laboratory result. Quality Improvement Coordinator/Hospital Coordinator Coordinates the quality improvement/performance excellence initiatives for the HIV/AIDS Program. Works with management staff to assess progress made toward established objectives based on identified key indicators. Provides technical assistance in achieving established goals, objectives and outcomes. Manages the HIV/AIDS Program’s implementation and progress in the area of process management and problem solving. Provides consultation to the management staff in mapping processes and the use of problem-solving techniques and tools. Reviews completed processes and provide feedback concerning work/client flow, procedures and systems. Monitors completion of these according to identified objectives. Meets regularly with staff to ensure responsibilities/expectations are achieved.Trains case management staff/contracted providers on eligibility look up in the HealthCare District Database and CHC System in order to research clients’ eligibility determination. Trains case management staff/contracted providers on assisting clients with the eligibility process, such as completing applications for Medicaid, Medicaid Managed Care and/or Health Care District. Provides consultation services between the Palm Beach County Health Department and local hospitals. Provides assistance to improve management capabilities in facilitating the exchange of medical histories, medical records, discharge plans and other pertinent patient information. Assists in identifying areas of potential improvement in the coordination operations of the exchange of medical records, discharge plans and other pertinent patient information. Provides patient advocacy, information and referral, as needed. Works closely with local case management agencies to ensure the smooth admission and/or discharge with the patient’s case management agency. Coordinates and/or provides training to the staff. Designs, implements and monitors program quality and determine improvements needed to ensure program effectiveness and efficiency. Advises and assists other/division, center and program staff with the development and implementation of HIV Prevention Program/activities and Patient Care Quality Assurance activities, as necessary. Networks and coordinates with community organizations and agencies involved in HIV Prevention Programs and Patient Care Quality Assurance activities. Serves on committees, task forces and advisory boards, as needed and as related to Patient Care Quality Assurance. Assists with Quality Assurance projects, as needed. Provides oversight, technical assistance, training and consultation to the supervisor of the on-site peer mentors and case managers consultants. Assists the supervisor design, implement and monitor program quality and determines improvements needed to ensure program effectiveness and efficiency. Meets regularly with supervisor to ensure responsibilities/expectations are achieved. HIV/AIDS Perinatal Linkage Nurse Provides consultation and technical assistance to nurse in the planning, implementation and delivery of care to HIV pregnant women and their newborns. Exercises independent judgment in the implementation of policies and procedures, which have significant impact on the HIV Perinatal Program. Plans and develops HIV Perinatal Linkage Program bases on epidemiological data and agency and community needs. Designs, implements and monitors program quality and determines improvements needed to ensure program effectiveness and efficiency. Prepares and disseminates required reports. Networks and coordinates with community organizations and agencies involved in HIV positive pregnant women. Services on committees, task forces and advisory boards, as needed and as related to HIV maternity and infant care. Supervises and evaluates the performance of the Perinatal Linkage Nurse, including the selection, orientation and training in methods required for performing an effective job. Early Intervention Staff Provides training, consultation, and technical assistance to Early Intervention staff to maximize and enhance EIS efforts, including but not limited to, record searching and verifying locating demographic information; performing pre- and post-test counseling, including partners; performing outreach and conducting risk reduction counseling to better link clients into medical care. Performs data entry activities in STARS, CAREWare and Provide Enterprise to assist in the monitoring and tracking of the EIS program. Monitors progress in the billing of Ryan White Part A services for the EIS program and ensures that staff is meeting program goals. Networks and coordinates with Department of Community Services staff in assessing the goals of the Ryan White Part A Funded Early Intervention Services Program. Supervises and evaluates the performance of the Early Intervention Services Staff, including the selection, orientation and training in methods required for performing an effective job. Ending the Epidemic: Rapid Entry to Care Ensure grant compliance with the expansion of Palm Beach County’s Rapid Entry to Care (REC) Center launch within the county health department. Serve as liaison between the county health department and Palm Beach County’s Community Outreach Response and Engagement (CORE) Teams. Develop and implement REC Center protocols and procedures to ensure clients are seen at the county health department Infectious Disease Clinics (IDC) within the grant required hours. Support the utilization and expansion of telehealth services, as needed, across the continuum of HIV care, in order to meet grant requirements. Expand peer community health initiatives. Attends meetings, conferences, trainings and in-services, as needed. Performs other duties as assigned.
