Seeking a Social Services Director for our 49-bed skilled nursing facility in South Tampa. You will manage the department to assure that assessments, discharges and psychological needs of residents are met. Understand MDS and Care Plan process. Accurately assess, document and provide interventions in the areas of hearing,speech,vision,cognitive patterns, mood, behavior and customary routine discharge and communicate interventions to the care team.
Coordinate and interface with Hospice, Ombudsman, home area, and coordinate transportation. Understand, comply & promote rules regarding all resident rights, promote positive relationship with residents visitors, and regulators.
Minimum requirements: Four year degree & nursing home experience preferred. Possess proficient computer skills.
