Job Summary:

Open position available - Perfect for a seasoned RN to be a Life Care Manager Consultant Advocate!

We are NOT a home care agency so there’s no hands-on care. We pay for all time worked; in-person with clients, at doctor’s appointments, phone calls with clients, family members, and care providers, coordinating services, writing assessments and progress notes, plus travel time and mileage reimbursement to/from your client visits. Must reside in and see clients in Pinellas County, FL, and be familiar with our local community resources.

Advanced Senior Solutions is a well-established private pay Aging Life Care Management agency serving seniors and disabled adults for 24 years. Our Life Care Managers assess needs, develop a plan of care, provide professional recommendations, arrange for care services, oversee care, and most importantly be strong advocates for our clients. They are proactive, problem solvers, thrive in sometimes stressful situations, and love challenging complex cases.

We provide you with clients, orientation, and office support and many years of Life Care Management experience for you to rely on. Start with a few clients and build up to 20-40 billable hours a week. Your schedule is up to you and the client as to when you see them. We have a fantastic team of Life Care Managers who can cover your cases while you take time off.

Requirements:

- Must have an active RN license in Florida with at least 10 years of experience in any of the following areas: home healthcare, hospital or insurance case management, with care coordination or discharge planning experience working mainly with seniors and disabled adults.

- CCM or CMC preferred.

- Already have or are willing to get L2 fingerprinting on file with AHCA.

Typical examples of duties:

- Perform assessments, develop detailed care plans, and offer professional recommendations.

- Arrange for community services and oversee those services.

- Advocate for the client whether they are at home, in assisted living, hospital, rehab, or nursing home.

- Perform nursing visits for medication management, vital signs, and disease management/education.

- Attend doctor’s appointments, care planning meetings, family meetings, etc.

- Provide crisis intervention, emotional support, and transitioning assistance.

- Research, recommend, and tour facilities with clients who need placement to help them make a well-informed decision. (We are not a free placement agency so we do not take any commissions).

- Life Care Management also includes assisting clients with other needs such as sorting/organizing mail and paperwork, bill paying assistance, reducing/avoiding scams, activating long-term care insurance claims, arranging for and overseeing home repairs/maintenance, estate sales, appraisals, moving companies, etc.
