We want you to apply!Job Summary and QualificationsThe Clinical Rehabilitation Liaison contributes to high-quality, patient-centered care by identifying candidates for inpatient rehabilitation and initializing timely, appropriate patient transfers. In collaboration with the Rehab Medical Director, the Clinical Rehabilitation Liaison ensures appropriate patient placement through clinical assessments and screening. The Clinical Rehabilitation Liaison serves as an inpatient rehabilitation expert between internal and external referral sources and inpatient rehabilitation operations. The Clinical Rehabilitation Liaison is responsible for establishing relationships with case managers, physicians, payors, and other referral sources.What you will do in this role:

You will assist and coordinate intake and pre-admission screening processes. You will document evidence of rehab admission criteria within established timeframe.

You will perform on-site and off-site clinical evaluations to assist Rehab Medical Director in establishing patient eligibility for admission.

You will provide information to patient/family/caregiver related to inpatient rehab and program features. You will communicate expectations and requirements for participation in the rehab program and conduct or coordinate tours of the rehab unit.

You will obtain information relating to patient resources and healthcare insurance benefits.

You will demonstrate clinical knowledge of rehab admission criteria to appropriately identify and qualify rehab patients.

What qualifications you will need:· Graduation from an accredited school in a clinical specialty to include nursing and/or rehab therapy· Must be licensed as a Registered Nurse (RN) or Licensed Therapist (PT/OT/SLP) in the state which you practice and/or hold an active Compact/Multi-State RN license· A current American Heart Association (AHA) or American Red Cross Basic Life Support (BLS) Health Care Provider is required· A minimum of 2 years recent clinical experience required· Experience in an inpatient rehab setting preferred· If an RN, CRRN preferred or must obtain within 2 years of accepting positionOrange Park Medical Center has provided quality healthcare services since 1974, giving patients access to highly-trained physicians and advanced technology.
