We want your knowledge and expertise!Job Summary and QualificationsThe Quality Coordinator is Responsible for the coordination and facilitation of performance improvement activities within assigned patient populations/groups and medical staff peer review activities. Serves as a resource person to assigned performance improvement groups in areas of regulatory standard compliance, performance measurement and education. Coordinates selected JC, CMS and HCA required measure reporting systems.Interprets data and shares interpretation concurrently with Medical Staff, Patient Care Services staff, VPQ and other direct care givers to promote compliance with outcomes expectations. Assists in the development and monitoring evidence-based best practices. Participates in statewide quality efforts as assigned to include Florida Medical Quality Assurance Inc. (FMQAI), TJC, Core Measures, Leapfrog, Institutes of Healthcare Improvement (IHI) and other applicable programs. Assists to coordinate, promote and maintain house-wide patient safety initiatives. Reports/participates in Team meetings, Patient Safety and Performance Improvement Council, applicable Medical Staff committees, and other committees as assigned. Works to expand knowledge of quality bringing new ideas and concepts to HCA Florida St Lucie Hospital. Provide support to departments overseen by the Vice President of Quality Management such as Risk Management/Patient Safety, Infection Control, Quality, and Medical Staff.

Acts as an expert resource to physicians, other managers, and patient care givers to consistently improve outcomes and performance. Develops creative learning tools and mechanisms for monitoring trends.

Assists in the development and monitoring of performance improvement activities throughout the hospital. Works in a collaborative manner with all departmental managers, staff, Case Management, physicians, and other allied health professionals. Leads PI processes within the individual departments as assigned. Proactively leads performance improvement initiatives including education as they related to outcomes management, including the coordination of evidence-based best practices; works with direct care-givers to improve compliance; and other follow-up data to support compliance with performance outcomes. Assists in tracer activities. Makes daily rounds in all areas assigned as required. Provides evidence-based research and articles to promote best practices, specifically as they relate to assigned measure set. Succeeds in helping to improve performance.

Assists to coordinate, promote and maintain house-wide patient safety initiatives. Promotes and

maintains compliance with TJC standards and elements of performance, National Patient Safety Goals; CMS Conditions of Participation, AHRQ and Leapfrog. Monitors compliance through tracer and periodic review activities. Attends assigned safety meetings and task force committees. Assists with the coordination and development of safety awareness education activities for staff and physicians. Develops graphs and monitoring techniques to trend variances and progress towards safety goals; (conducts research to provide LIPS and clinicians information to support proactive change).

The Quality Coordinator has full access to all medical records to review and identify real or potential risks, key issues and/or trends. Collects and or validates data for assigned measurement analysis in patient care areas assigned. Identifies patients who are included in assigned measure set, measures appropriate care. Reviews and acts on all data collected to facilitate best practice and positive outcomes, including performance outcomes. Assures that data abstraction is timely and correct.

Coordinates retrospective data collection to ensure COMET/Core measures, STS, ACC-NCDR,GWTG and other databases are appropriately maintained. Validates COMET/Core Measures and other data sets as assigned; coordinates on time submission, reporting, and maintenance of STS, ACC-NCDR, GWTG and other data that is abstracted by others. Promotes and facilitates evidence-based practice and creative performance improvement initiatives that are congruent with best practices and which meet or exceed outcome benchmarks.

Interprets data and shares interpretation concurrently with Medical Staff, Patient Care Services staff, and other direct care givers to promote compliance with outcomes expectations. Concurrently networks and communicates variances based on abstraction findings. Facilitates correction of practice as indicated. Prepares information and reports for committee meetings and provides data to demonstrate St Lucie’s current performance in assigned measures. Provides resources and/or research to promote compliance with best practices. Communicates effectively during committee reporting; conducts crucial conversation to address performance variances. Builds trust and credibility with medical staff and patient care team as measured by meeting performance expectations, demonstrated collaboration and program compliance

Participates in statewide quality and accreditation efforts as assigned to include Florida Quality Assurance Inc. (FMQAI), AHCA, CMS, TJC, Core Measures, GWTG, Leapfrog, Institutes of Healthcare Improvement (IHI) and other applicable programs. Completes assignments as they are related to external collaborative and public reporting. Collaborates to assure data is submitted on time. Provides measure set data as requested to complete reporting. Leads an assigned measure set collaborative initiative

Prepares/reports/participates in Team meetings, Patient Safety and Performance Improvement Committee, monthly division Quality Monthly Operations Report (QMOR), applicable Medical Staff Committees such as Peer Review, Credentialing, Departmental Meetings, etc., and other committees as assigned. Responsible for OPPE/FPPE of Medical Faculty Members and Advanced Practice Professionals. Participates in committee activities as assigned. Develops reports as indicated. Assists in maintaining timeliness of reviews. Develops creative graphs and displays to showcase data for ease in understanding. Assists in the preparation of reports for the MEC and Board of Trustees

What qualifications you will need:

Registered Nurse - Currently licensed as a registered professional nurse in the state in which he or she practices, in accordance with law and regulation Preferred

Bachelor's degree Preferred

3+ years of experience in acute hospital setting Required

CPPS: Cert Prof Patient Safety or Certification in Quality (CPHQ) Preferred
