Payment is made at the end of each 12-month enrollment period and made directly to the participant’s lender.

Enrollment and payment of awards is based on available funds.

MINIMUM QUALIFICATIONS :

Valid licensure as a Registered Nurse.

Current FDVA employee.

Willing and able to work weekends and holidays as required.

Excellent Customer Service and Interpersonnel skills

Degree in Nursing from an accredited college or university

Long-Term Care, skilled nursing home experience

Basic or better computer skills to include Microsoft Suite.

Able to communicate effectively in English: speaking, understanding, reading and writing.

Experience with MDS

Experience in Care Planning

Experience in ICD-9 Coding

Experience with electronic medical records

Successful completion of a current Level 2 Background Screening to include,but limited to finger printing and a Drug Test

PREFERRED QUALIFICATIONS :

Preference will be given to candidates with the following qualifications:

MDS Certification

Experience in Regional MDS position, multi-facility role

Experience with Matrix Care electronic medical records

POSITION DESCRIPTION :

Coordinates and oversees all functions of the FDVA Homes Programs MDS Assessment process to assure compliance with Federal and State requirements. Performs random audits to assure prompt completion and transmission of the required MDS assessments.

Availability to MDS Coordinators and Interdisciplinary Team members for problem solving related to the MDS assessment completion, transmission and care planning process.

Reviews and updates interdisciplinary team members with current standards of practice in the MDS process for regulatory guidance. Participate in the Quality Validation Surveys and complete facility on-site visits as needed.

Develops and provide educational programs to the Interdisciplinary team members.

Coordinates with Billing to assure accurate Resource Utilization Groups (RUGs) and the billing process.

Performs other duties and projects as assigned.

Other job-related requirements for this position: Must comply with FDVA Smoking Policy including compliance with those facilities designated as “Tobacco Free Campuses”. Follow all FDVA Policies and Procedures, Homes Program Standards and Procedures. Must be available, if requested by management, to respond to disasters such as Hurricanes affecting the facility. Periodic local and overnight travel many/will be required, if applicable. Exposure Control Rating-Level I potentially at Risk. Must have the ability to perform the following physical tasks continually or frequently throughout the work day- mobility, balance, climbing, crouching to bending at the knees, grasping, kneeling, crawling, pushing ability, to lift up to 30 pounds, reaching above head, reaching with arms extended, standing, stooping or bending at waist, turning or pivoting, twisting at the neck, waist or knees, and ability to hear and talk in person or on the telephone, vision for close work.
