We are looking for a compassionate and organized Care Coordinator to support our team and the families we serve. In this role, you’ll find and help connect caregivers with patients, making sure schedules run smoothly and care is provided when it’s needed most. What You’ll Do:
Build and maintain weekly/monthly schedules for caregivers, ensuring every patient has the right support.
Recruit and match caregivers with patients based on skills, availability, and compatibility.
Welcome, onboard, and orient new caregivers so they feel confident and supported.
Coordinate patient services, including "Start of Care" visits, and make sure documentation is completed on time.
Stay in touch with patients, caregivers, and staff to keep communication clear and schedules updated.
Track and share updates on hospitalizations, discharges, or missed visits.
Help with referrals and connect with healthcare partners when needed.
Step in to support the team with other coordination or administrative needs.
What We’re Looking For:
Experienced scheduler preferably in Home Health or medically equivalent.
Someone who loves being organized and can manage schedules and details with ease.
A great communicator who is comfortable with excessive phone usage during working hours.
Friendly and adaptable personality with emphasis on strong people and diverse personality management skills.
Comfortable working with all types of patients.
Basic computer and office skills to keep everything on track and compliant.
A caring, patient, and team-focused attitude.
Ability to problem solve individually while following policies and procedures provided by the company.

Office-based work with phone and on the computer use, with occasional coordinating/delivering of supplies, or meetings. On-call is required with compensation.
