We want you to apply!Job Summary and QualificationsAs a Multisite Oncology Patient Navigator, your primary responsibility is coordination and follow-up of patient care for assigned Oncology patients. This includes serving as a resource to inpatients, outpatients, nursing staff, and physicians while assisting with implementation of growth strategies and process improvements to maintain accreditation by regulatory agencies.What you will do in this role:

Serve in a leadership role with the Oncology team to enhance the quality of care and patient outcomes for Cancer patients.

Assist patients and families in understanding their diagnosis, treatment options, and resources available.

Follow patients throughout the care continuum, including inpatient admissions, and collaborate with inpatient care management resources.

Is an educational resource to patients, family, and staff.

Coordination of care of the oncology patient with the medical staff through professional relationships with physicians and other medical/support staff to ensure the best possible coordination of the care throughout the course of their disease.

Maintain current knowledge of clinical improvement strategies and outcomes management to assist with the maintenance and growth of the program.

Coordination of multi-site Cancer conferences to discuss diagnosis and treatment of identified patients.

Coordination of process improvement activities which includes assignment to various departments for required follow up to improve performance outcomes.

Work with the Cancer Registry staff and others to collect data, track outcomes, and support strategic planning processes.

Assist with outreach efforts to establish and maintain positive working relationships in the community.

What qualifications you will need:

Graduate of accredited program in nursing, Radiation Therapy, ARNP, or PA.

Three to five years of progressive clinical experience in oncology

Excellent leadership, organizational, and communication skills both verbal and written.

Ability to implement professional and community based education programs.

Self directed with excellent problem solving, decision making, and critical thinking skills.

Computer skills including but not limited to Microsoft Office, Excel, Power point, and the ability to utilize these to abstract, interpret,and present data to multiple audiences
