Attend HIV clinics at the Alachua County Health Department 5 days a week. This position is a 5 day a week, 40 hours a week position. Be available to the attending physician. See clients in clinic, both case managed, and non- case managed. Document interactions with client in client’s electronic medical record. For case managed clients, provided written documentation on interactions to appropriate case manager. Provide social service information/referrals as needed. Will see and assist clients in the PrEP clinic as needed. Will see and assist clients in the Test and Treat clinic as needed. Receive and coordinate medical referrals. This includes but is not limited to referrals from HIV clinics, but any other Health Department clinic that a HIV client may attend. Document efforts in handing referrals in client’s electronic medical record. If case managed provide written documentation on referrals to the appropriate case manager. Completes EARS daily. Attend and participate in staff meeting and training programs. Performs other duties as assigned, this includes but is not limited to Emergency duty. During Emergency duty: works during and beyond normal work hours or days in the event of emergency operations. Functions required include but are not limited to Special Needs Shelters, Red Cross Shelters or other emergency functions. At all times maintains confidential information such as social security numbers, medical information, and other confidential data in a secure manner. The incumbent has a need to access records containing SSNs for the performance of their duties. Respects the confidentiality of related issues by not discussing with unauthorized staff. Ensures discussions held between staff cannot be overheard by unauthorized staff. Reports suspected breaches of security to supervisor immediately. Required Knowledge, Skills, and Abilities: Knowledge of social, economic, rehab or health care services, administration or planning. Knowledge of methods of compiling, organizing and analyzing. Ability to develop program manuals, policies, procedures, standards, rules. Ability to conduct research. Ability to plan, organize, and coordinate work assignments. Ability to communicate effectively, orally and written. Ability to establish and maintain effective working relationships with others. Ability to use Microsoft Office.
