We want your knowledge and expertise!Job Summary and QualificationsAs an Advanced Clinical Pharmacist Medication Reconciliation, you will be responsible for the safe, effective, evidence-based, and cost-effective selection and utilization of medications for all patients. You will serve as a patient advocate when planning and implementing medication therapies and collaborating within the multidisciplinary team.What you will do in this role:
You will work directly with the certified pharmacy technicians provide a streamlined medication reconciliation transition into the hospital.
You will ensure the medication history and reconciliation is completed according to established policies, procedures and protocols.
You will provide ongoing mentoring and training to ensure the competency of medication history and reconciliation staff.
You will ensure safe, appropriate, cost-effective medication therapies for patients, according to the established policies, procedures, and protocols.
As needed, you will provide recommendations and/or literature to prescribers supporting the use of formulary agent, as well as, education to clinicians and patients regarding medication management.
What qualifications you will need:
B.S. or Pharm D. degree in Pharmacy is required.
Graduate of an accredited PGY-1 pharmacy residency program is preferred.
Graduate of an accredited PGY-2 pharmacy residency program is preferred.
3+ years hospital pharmacy supervisory experience is preferred.
Maintains current pharmacist licensure is required.
National Provider Identifier (NPI) is required.
