Your Specific Responsibilities:

This is a professional nursing supervisory position, with responsibility for appraisals, planning and directing work with authority for hiring and taking disciplinary action or effectively recommending such actions. This position is the second level of authority in the chain of command for nursing in the clinic at the DeFuniak Springs site and will provide backup coverage for the Senior Community Health Nursing Supervisor. This position will have the responsibility to provide nursing services through Primary Care, Prenatal Care, Family Planning, Immunization and STD clinics as needed. The person in this position will be performing tasks involving sensitive information or requiring a high level of trust for other reasons. This position is a key custodian for the laboratory, medication, record rooms and other assigned areas, position will adhere to the Department's Information Security policies, protocols, and procedures and responsible for the supervision of staff Including appraisals, planning, and directing their work, approving leave, having the authority for hiring, and taking disciplinary action or effectively recommending such actions.

This position will follow Florida Department of Health, Florida Department of Health Walton and Walton Community Health Center policy and procedures. This position is a key custodian for assigned areas.

This position will provide patient-centered interactions:

This position may assist in the provision of care coordination:

Link patients with community resources to facilitate referrals and respond to social service needs.

Track and support patients when they obtain services outside the practice.

Obtaining test and referral results and communicating with community organizations, health plans, facilities, and specialists.

Follow-up with patients within a few days of an emergency room visit or hospital discharge.

Communicate test results and care plans to patients/families.

Assures that those supervised function within position expectations and in a manner that complies with agency standards, supports providers, and provides for optimal efficiency. Functions as a coach and mentor to current and newly hired Licensed Practical Nurses (LPNs) and Registered Nurses (RN’s). Takes an active role in the orientation of those supervised. Is responsible for the development of an on-going revision of orientation plans for those supervised. Assures that new employees have an adequate orientation and support during the first few months of employment. Is responsible for the nursing scheduling/staffing needs of the DeFuniak Springs location. Monitors/adjusts staff schedules to meet clinic need. Coordinates staff schedules with medical providers and other supervisors to meet needs of all clinics and locations. Works closely with providers, staff and supervisor to assure alignment of operations between the DeFuniak Springs site and all other clinic sites.

Provides supervision of clinic-based nursing staff to include required evaluations, counseling, and discipline. Participates with monitoring/QI-QA activities to assure compliance with program guidelines and regulations. Develops and puts into place corrective action plans as needed, re-evaluates changes for improvement and adjusts actions as needed. All activity is documented in an approved format. Encourages staff participation in educational and personal growth activities. Holds staff meetings/ counseling sessions as needed and documents these in an easily usable/retrievable manner. Is a positive role model to co-workers and other DOH staff.

Position is responsible for following agency guidelines, policies and procedures while providing generalized nursing services to clients. These services include clinics for immunizations, family planning, prenatal care, primary care, and sexually transmitted diseases. Demonstrates special nursing procedures and techniques applicable in a variety of medical specialties and teaches the purpose, use, care, and results of utilizing special equipment in the treatment of patients. Has the ability to "step in" and function and/or troubleshoot as needed to assure clinic flows efficiently. Provide generalized nursing services including nursing assessment, nursing diagnosis, counseling, case management, education and follow-up. Assist medical providers with minor surgical procedures, colposcopies, or other complex procedures in the clinical setting. Assists physician(s) and ARNPs as needed by obtaining vital signs, reviewing forms for accuracy and completeness, preparing exam rooms and assisting with exams as needed and/or administering medications, screening for vision and hearing where indicated. All nursing care is to be documented in the electronic health record. Provided services will be captured on Super Bill and/or in HMS.

May provide client tracking and care coordination/case management tasks for physicians and ARNPs. Will work with supervisor and other applicable staff to ensure that clinic is well stocked with supplies and/or medications. May provide back-up for the telephone triage nurse. Assures that clinic and exam rooms are clean, well-stocked and in good operating order. Assures that annual equipment checks / calibrations are completed for all diagnostic equipment in the clinic at DeFuniak Springs.

Works closely with Medical Director, Nursing Director and supervisor to assure that laboratory operates within regulatory guidelines. Assures adequate educational opportunity for those supervised. This position will also be responsible for collecting laboratory specimens from clients and preparing lab specimens for transport in compliance with agency requirements of Walton County Health Department. Laboratory duties may extend to: Use of the centrifuge for separation of blood specimens as indicated, Data entry for lab testing may be manual or electronic (dependent upon lab) as is the tracking of results and specimen packaging/shipping. Assures tracking of laboratory specimens with dates sent out and dates results received.

Will be assigned the afterhours on-call for client phone calls, abnormal lab results; epidemiology, notification and follow ups.

Provide community health education and community-based clinics as requested. Has a can-do approach to problem solving. Will actively participate in committee and team activity as permitted or requested by supervisor. Attend supervisory and staff meetings.

Attend conferences and training sessions to enhance job skills. Takes a pro-active position in personal and professional development. May be required to utilize personal vehicle for travel. Complete employee activity records and reports according to agency standards. Will Perform other related duties as assigned. Attends and participates in monthly general staff meetings and departmental meetings.

Completes all job-related assigned trainings, Completes Employee Activity Reports and submits timesheets according to agency standards and performs other duties as assigned. Assigned purchasing card responsibilities and will adhere to purchasing card policies and procedures.

Qualifications:

Minimum – 0-12 months supervisory experience, Licensure as a Professional Registered Nurse, current state driver’s license, current CPR/BLS certification HIV 501 certification
