What You’ll Do:
Care Coordination – Match caregivers to clients, oversee care plans, and ensure top-quality service.
Client Advocacy – Be a voice for our clients, helping them access the resources they need.
Caregiver Support – Guide and supervise caregivers to maintain excellence in care.
Compliance & Documentation – Keep accurate records and ensure all care follows regulations.
Resource Coordination – Connect clients with community services for a fuller, healthier life.
Hands-On Support – Step in as a Personal Care Aide when necessary, making sure no client is left without assistance.

Minimum Job Requirements:
Authorization to work in the United States
Personal Care Aide Certification (we offer free PCA certification!) or unexpired Certified Nurse Aide license
Valid, unexpired driver’s license
Must have own vehicle with appropriate auto insurance coverage
Negative PPD test or negative chest X-ray if PPD was positive
Unexpired CPR certificate or higher
Other Important Job Requirements:
Good command of the English language, written and spoken (bilingual a plus!)
Proficiency with Microsoft 365 apps and Google apps
Experience with electronic health record system
Ability to prioritize tasks with considerable efficiency
Excellent customer service skills
A criminal background check will be conducted once a conditional job offer is given.
Job Type: Full-time, Salaried/Exempt
Schedule:
8am-4pm and PRN (as needed)
Monday to Friday, may include weekends as needed
Compensation: starts at $47,840.00 per year
