As a Direct Admission Center Patient Placement Coordinator, you'll manage in-patient bed placement requests, ensuring timely placements and smooth direct admissions flow. You'll support Pre-Admit issue resolution, train staff on bedboard systems, and coordinate efforts among Patient Placement, Care Management, and Admitting teams. You'll also maintain accurate records and drive process improvements.Responsibilities:

Triage and escalate issues to the Administrative Nursing Coordinator in order to support flow and the safety of patient placement.

Ensure complete and accurate Pre-Admit Tracking information.

Adjust cancelled admissions by verifying changes with admitting, notifying the ANC/unit, and enters changes or cancellations into the Pre-Admit Tracking screen.

Make effective decisions within the scope of delegated authority

Serve as the main contact with Teletracking for the Pre-Admit application with respect to trouble shooting, upgrades etc

Prioritize and place patients into the appropriate unit assignment by using critical thinking skills, keeping within the department policies and guidelines, and maintaining sensitivity to each individual customer.

Oversee the flow of patients from ED, outlying facilities, physician offices, clinics and home to insure that the goals of an efficient and seamless entry into the facility occurs

Work as a liaison with MedCall to insure timely transfer of patients

Maintain customer confidence and protects the organization by keeping all information confidential.

Schedule: Rotating 12 hours shifts; various start times between 5:30am to 10:30pm. Rotating days, weekends, and holidays.

Completion of of High School diploma or equivalent and post-high school education desired.

2 years of minimum experience in patient placement, patient access required.

Must be able to demonstrate strong critical thinking skills, multi-tasking ability and customer service skills.
