The Referral Specialist utilizes reports to identify open specialty referrals requiring follow up with specialists’ office to determine status of patient appointments. The Referral Specialist helps ensure patients’ timely access to outside specialty services by identifying any barriers to patients being seen and coordinates with patients and the ordering Wheeler providers and care team, as needed, to address and resolve barriers to care. The Referral Specialist maintains accurate tracking records in the electronic medical record and obtains and imports external specialists’ records into patients’ medical records. The Referral Specialist reviews specialists’ records and performs data entry and internal routing within the electronic health record to ensure optimal coordination of primary and specialty care.Hourly Rate $21.00 Bilingual in Spanish and English is required. Bilingual stipend: $2,288 EDUCATION AND EXPERIENCE/QUALIFICATIONS High School Diploma is required. Proficiency in reading, handling, and scanning electronic medical and/or other records or documentation is required. Must be capable of working flexible hours and have access to reliable transportation. English / Spanish bilingual ability is required. At least one (1) year previous related experience in a health care setting, including knowledge of medical procedures and terminology. ESSENTIAL DUTIES AND RESPONSIBILITIES
Work closely with internal and external healthcare providers/care teams and office staff to ensure patients receive timely access to outside specialty services to ensure appropriate diagnosis and well-coordinated treatment for health center patients.
Utilize electronic tools including reports and excel to track, identify and prioritize open referrals requiring follow up.
Perform follow up activities to determine status of referral and identify and communicate to the patient’s care team any barriers to care requiring resolution such as missing contact information, insurance issues, lack of transportation.
Provide appropriate clinical information to specialist office as needed.
Educate patients related to importance/requirements of referrals, referral sites/locations and/or appointment specifics.Maintain accurate tracking and follow up records for referrals within the electronic health record. Communicate with patients and internal/external providers/care team and office staff regarding referrals. Review specialists’ records and perform data entry (results, UDS clinical measures, closing of referral) and send specialists’ records via the electronic health record to the ordering provider to ensure optimal coordination of primary and specialty care.Perform data mining in external medical record systems to identify referral status and support coordination of care, UDS clinical measures, and import/data enter information into Wheeler’s systems.Demonstrate excellent attention to detail and compliance with HIPAA and other state/federal privacy regulations and Wheeler privacy policies when handling patient information and communicating with external individuals. Achieve established productivity standards as set within the Central Records Unit.Demonstrate the ability to work as part of a team and contribute to the overall effective operation of the Central Records Unit and coordinate across Wheeler departments to support the overall mission of the organization.Continue to develop knowledge and understanding about the history, traditions, values, family systems, and artistic expression of groups served as well as uses appropriate methodological approaches, skills, and techniques that reflect an understanding of culture.Wheeler Clinic is an equal opportunity employer.
